If you do not have access to the patient portal, please call the office to provide an email address and any of our office staff members will be happy to set up your account.

By mail, sending a check with the statement stub or with your credit card information written in the appropriate fields
By phone with a credit card, by calling 614-784-UROL (8765).

Payment plan options are available on a case-by-case basis. Plans can be discussed further with Capital Urology staff by phone. Please contact our office at (614) 784-UROL (8765).
*If you are paying on behalf of a spouse, family member, etc., please provide the name on the statement, date of birth, and the address so we can locate the patient’s chart quicker for you.*
